Long Beach State Students:
Over the past several weeks Associated Students, Inc. (ASI) has been evaluating the use of the University Student Union (USU) and Student Rec & Wellness Center (SRWC) through focus groups with various student stakeholders. We are looking to learn what students like about the USU and SRWC now and what students want to see in the future to better serve their needs. In an effort to expand the conversation, we are launching a student-body-wide survey to make sure all student voices are heard.
As appreciation for your participation in this approximately 10-minute survey, we will also be entering you to win one of the many thank you prizes in our opportunity drawing.
- 1 of 1 - Apple iPad
- 1 of 2 - Apple AirPods
- 1 of 1 - Nintendo Switch
- 1 of 4 - $25 Bookstore Gift Cards
- 1 of 4 - $25 Coffee Bean Gift Cards
- 1 of 8- $15 Amazon Gift Cards
Once you have completed the feedback survey, you will be asked to enter your name and CSULB email address (@student.csulb.edu) to make you eligible for the prizes.
The survey will close at 11:59 p.m. on Friday, Dec. 6, 2019. You may access the survey using the link above or below.
TAKE THE SURVEY NOWWe thank you again for taking the time to participate in this important project for the Long Beach State community. ASI is always striving to improve our services to the student body and we can’t wait to see your feedback! Go Beach!

Additional Details:
All information that you provide is strictly confidential. No individual responses will be identified. Only
currently enrolled CSULB students are eligible for prizes. Winners will be notified by ASI directly via email
once the survey period is closed and entries are processed. ASI student government leaders and staff are not
eligible in the opportunity drawing, though are still encouraged to participate in the feedback component.
Nearing the USU’s 50th year anniversary and the SRWC’s 10th, Associated Students has retained the services of a
nationally recognized, independent consulting firm, Brailsford & Dunlavey, to administer a facility use and
programmatic needs assessment for both facilities. Should you have any questions about this process, please
contact survey@programmanagers.com.
